Allergies, Dietary Restrictions, or Other Medical Conditions

Briefly describe the student's food allergies and their severity, any other dietary restrictions they may have, and any medical conditions that you'd like Longy staff and instructors to be aware of in the space provided below.

Liability Policy

To the fullest extent permitted by law, I, the legal guardian of this student, shall defend, indemnify and hold harmless Longy School of Music, its trustees, directors, officers, employees and agents, from and against all losses, claims, suits, actions, damages and expenses (including attorneys’ fees), arising from (Contracted Party) breach of this Agreement, or (Contracted Party) acts or omissions in performing services under this Agreement, except solely to the extent such Claim is directly and proximately caused by the negligence or willful misconduct of Longy School of Music.

Attendance Agreement

Attendance at all Side-by-Side rehearsals, concerts, and sessions is mandatory. If your child cannot attend a session, you must submit an absence request online at least 24 hours before the event. Consistent participation is essential for individual and group musical development—multiple unexcused absences may affect continued participation in the program.

Walk Home Form (Optional)

I hereby grant permission for my child to walk to and from Longy School of Music’s Side-by-Side rehearsals independently. I understand that this means my child will be solely responsible for their own safety and actions during their commute to and from the rehearsal venue, located at 27 Garden Street, Cambridge MA, 02138.
By providing this permission, I acknowledge that I am relinquishing Longy School of Music from any responsibility for my child during the commute times. I understand that my child will not be under direct supervision by any authorized adult from Longy School of Music during these periods.
I have ensured that my child is aware of the necessary safety precautions and guidelines for walking to and from rehearsals, including but not limited to:

Using designated crosswalks and pedestrian paths.
Avoiding interaction with strangers.
Carrying identification and emergency contact information.
Contacting emergency services or a trusted adult in case of any emergencies.
